Medical Documentation and How To Obtain It:

State of Florida Advanced Medical Directives (Living Will):

To get an advance directive in Florida, you can download a form online from resources like the Florida Heath Finder website, your local hospital, or your doctor. You will need to complete the form and have it signed by two witnesses, at least one of whom is not a blood relative or spouse, and some forms may require a notary. Once signed, make sure to share the completed document with your healthcare proxy, doctor, and other trusted individuals.
You DO NOT need a lawyer to create an Advanced Directive in Florida.

State of Florida Do Not Resuscitate Order (DNRO):

A do not resuscitate order (DNRO) is a form or patient identification device developed by the Department of Health to identify people who do not wish to be resuscitated in the event of respiratory or cardiac arrest.  

Important!!!!

In order to be legally valid, this form MUST be printed on yellow paper prior to being completed. EMS and medical personnel are only required to honor the form if it is printed on yellow paper.

Flagler County Fire Rescue File of Life:

The "File of Life" program is locally organized by Flagler County Fire Rescue, and it will hold essential medical information, such as allergies, medications, and emergency contacts to assist all of our first responders in an emergency. The program is designed to be a "file of life" in the sense that it can significantly improve a person's outcome during a medical crisis by providing crucial information to responders who might otherwise not have it.

Program Basics:

  • How it works:
    • Residents fill out a card with their medical information.
    • The card is placed in a magnetic holder on the refrigerator.
    • A sticker is placed on the front door to alert responders to the presence of the File of Life.
  • Benefits:
    • Allows for faster and more appropriate medical treatment.
    • Ensures loved ones are notified.
    • Provides critical data to first responders upon arrival.
